Abstract

This article aims to share the experience lived by a multidisciplinary team providing care for people with hypertension and/or diabetes in the Family Health Care Strategy. The work was conducted during the process of Multidisciplinary Residency in Family Health Care, in which a team consisting of a nurse, a physical educator, a psychologist, a speech therapist, a physiotherapist, and a nutritionist shared the care for the people with these diseases, bringing about a more holistic view in the care process. The professional approach was jointly performed, and the center of the action was the user of the health service. Throughout this process, we could observe several possibilities in the share of this demand, as well as the need for a deeper discussion in the multidisciplinary field and in the field of mutual learning and for a strengthened relation with the user.
